Release of Liability
I, the undersigned parent or legal guardian, agree to release and hold harmless:
Darko Byrd Tennis Academy, its coaches, staff, volunteers, and affiliated facilities
from any claims or liability arising from injury, illness, or damages related to participation in academy activities to the fullest extent permitted under Georgia law.
Medical Authorization
I confirm that my child is physically able to participate in tennis activities.
In the event of an emergency, I authorize academy staff to obtain medical treatment for my child if necessary.
I understand that I am responsible for any medical expenses.
By allowing my child to participate in Darko Byrd Tennis Academy and Summer Camp programs, I authorize emergency medical treatment if necessary.
